Presenting complaint
The patient presented with a six-week history of progressive non-productive cough and exercise intolerance.
Imaging findings
CT revealed numerous well-defined, variably sized, round, soft-tissue attenuating nodules throughout all lung lobes. There was also undulant and contrast-enhancing thickening of the dorsal aspect of the trachea. Bronchoalveolar lavage and biopsy of the tracheal lesion were performed. Cytologic and histopathologic examination confirmed marked mixed, predominantly pyogranulomatous inflammation. No neoplastic cells were identified.

Key take-home points
Not all pulmonary nodules are neoplastic! They can also be granulomatous, for example due to inflammatory and/or infectious etiologies.
