Presenting complaint and brief history
presented with acute severe spinal pain and increased pelvic limb weakness and ataxia, with a markedly elevated CRP. These signs began about 6 weeks after a left L2-3 mini hemilaminectomy with fenestration, and had been preceded by a revision hemilaminectomy and a postoperative surgical site abscess.

Lytic lesions involve both vertebral end plates at T13-L1, L1-2, and L2-3, with the lysis most pronounced at L1-2. The L1-2 disc space is nearly completely collapsed, and L2-3 is narrowed. There is surrounding end plate sclerosis, most prominent at L2-3 and around the lytic regions at L1-2. There is mild contrast enhancement of the paravertebral soft tissues immediately surrounding the L1-2 disc space.
