Presenting complaint and brief history
Patient presented for pollakiuria. A mass cranial to the urinary bladder was identified on prior abdominal radiographs. The patient has a history of a mast cell tumor removal from the flank 3 years ago.
Diagnosis

Peritoneal carcinoma
Confirmed
Imaging findings
A large, well-circumscribed, heterogeneous, and vascular caudal abdominal mass measures 4.9 x 3.6 x 5.5 cm. It cannot be completely separated from the cranial urinary bladder wall and causes a concave indentation in the bladder. Multifocal blood flow signal is noted throughout the mass noted on color Doppler interrogation. Scant peritoneal effusion is present around the mass, and surrounding fat is mildly hyperechoic.
What happened with the patient
Lost to follow-up.
Key take-home points
Imaging can't always tell you where a mass comes from. When the origin is unclear and the answer matters for management, sample it.
