Presenting complaint and brief history
The patient is a 13-year-old spayed female domestic shorthair with chronic kidney disease, previous idiopathic hypercalcemia, recurrent lower urinary tract signs, and a progressive irregular urinary bladder wall lesion. The lesion showed progression despite prednisolone therapy. The patient is currently hospitalized for acute on chronic kidney injury and is improving with intravenous fluid therapy.
Diagnosis
Hides the diagnosis, imaging findings, outcome, take-home points and correlation photos so you can read the images blind.
Urothelial (transitional) cell carcinoma
Confirmed
Imaging findings
Marked irregular thickening of the cranioventral urinary bladder wall with complete loss of normal layering and strong internal vascularity.
What happened with the patient
Two ultrasound-guided fine-needle aspirates were inconclusive. Surgical biopsy confirmed the lesion's etiology. Partial cystectomy was deemed not a viable option as there would not be enough normal bladder left.
Key take-home points
- Partial cystectomy can be considered depending on the size and location of a bladder mass.
- If the ureterovesical junctions are affected (i.e. obstructed), ureteral reimplantations can also be considered.
